Know where to go to pick up your prescriptions.
As a non-profit cooperative, we know the cost of health insurance and health care are our members’ primary concerns, and we will always look for ways we can save money for our members. We’ve made changes to our pharmacy network that resulted in $1.1 million in savings that we have passed along to our members in rate decreases starting in January of 2019.
Please click the pharmacy directory on the right to make sure your pharmacy is still in network. Most of our members will not be impacted by the changes. That’s because our pharmacy network still includes many popular chains including Walgreens, Walmart, Kmart, Shopko, Costco, Meijer, Kroeger, Pick n’ Save, Bellin, Aurora, Thedacare, Skywalk Pharmacies (located within Children’s hospital locations), The Medicine Shoppe Pharmacy, Crivitz Pharmacy and some other independent pharmacies.
Now excluded from our network is CVS (including those located within Target stores), Sam’s Club and some local, independent pharmacies.
If you want to be sure about your pharmacy being in network, you can always contact our Pharmacy Benefit Manager, OptumRx, to check on the network status of pharmacies. That number is 855.577.6545. Are you interested in mail order for your prescription medications?
CGHC plans offer 90 day fills of certain maintenance medications for only two co-pays. You can sign up for mail order by visiting the OptumRx portal or calling the number on the back of your ID card. Click here for frequently asked questions and more information.
Access to this formulary does not guarantee any coverage details or benefits available; it only provides access to view the full list of medications that are available through Common Ground Healthcare Cooperative’s Prescription Drug Formulary. For a complete outline of covered benefits and details on medication coverage, please refer to your policy and the Schedule of Benefits provided in your enrollment packet. Some medications listed on the Prescription Drug Formulary may not be covered under your specific benefit. Where differences are noted, the benefit plan documents will govern. We reserve the right to add or remove prescription drugs from the formulary as deemed necessary.
