Envision Network – Available in all CGHC service area counties (Orange/Grey)
Rise Network – Available for small businesses in Washington, Ozaukee, Waukesha, Milwaukee, Racine and Kenosha counties (Grey)
First Health Complementary Network – Available with EPO Plus plans and for EPO members who seek urgent care services from a First Health Complementary Network provider when outside the Envision Network service area(Orange/Grey)
